How to submit the request



If you already employ a full-time caregiver and are receiving a long-term care benefit from the National Insurance Institute, please submit the form  "Request for cash long-term care benefit instead of services to a person employing a full-time caregiver" (Hebrew) directly to the National Insurance Institute.  

If you are planning to employ a foreign worker as a full-time caregiver – upon submitting your request to the Immigration Authority at the Ministry of Interior, please indicate on the request form if you choose to receive the long-term care benefit from the National Insurance Institute in cash or in services from a of a long-term care company.

Submitting a request for a foreign permit from to the Immigration Authority requires to fill out 3 forms:
  1. Request for a permit to employ a foreign worker.
  2. Employer's commitment
  3. Waiver of medical confidentiality

Appendices to be attached to the request:
  • Certificate for payment of fee.
  • Photocopy of patient's identity card + appendix (and of the "employer" if the patient is not able to fulfill his duties as employer of a foreign worker).
  • A guardianship certificate is to be attached if the patient is under the care of a legal guardian.
